I'd try Dextromethorphan, 60 mg., every eight hours for the pain.  Also, 
cannabinoids might help (Do you live in a Medical Marijuans State?)

Also, Lidocaine infusions, 4 mg per Kg, should help. Direct your Neuro to 
contact Dr. Christine Sang at the Massachusetts General Hospital in Boston.
