Title: RE: 837...Claim Frequency type code for HCFA-1500 claims

It is my understanding that NUBC has dropped code 6=corrected from the list of valid values as of October 2001.

You can download a pdf of the valid values for clm05-3 from http://www.nubc.org/guide.html by right-clicking on the Form Locator 4 … entry in the list of Other Resources.  Although I detest defaults, the value of ‘1’ makes sense to me.

Hope this helps.

Diane,

I went back to revisit this issue in the implementation guide.  The addendum refers you to code source 235.  Code source 235 again speaks to the UB specifications for type of bill position 3.  My conclusion from this is:

Even though the addendum removed the codes listed in the original IG, I believe that the original interpretation remains that we would expect to see:

1 = original claim
6 = corrected
7 = replacement
8 = void

No these are not currently found on the HCFA-1500 claim form.  My guess is that the logical default (which would be an individual organization's decision)  would be "1".

Can anyone clarify how element CLM05-3, claim frequency type code, in the
CLM Segment, Loop 2300, will be set for HCFA-1500 claims?
This is a required field, but it is not found on the HCFA-1500 claim form.
Is there a default value?
