This seems painfully obvious but can anyone direct me to an authoritative resource that clearly states that an Adjustment Reason Code that is listed as "Inactive for 003040" is also inactive for all subsequent versions ? It's the obvious assumption but I would prefer to see it from HHS, CMS, WPC, WEDI/SNIP, NMEH, etc...
