thanks to everyone -- so for documentation purposes, here's what i got
working: 


transcode -J extsub=0:0:0:1:0:255:255:0:0 \
          -i /media/mydvd \
          -y ffmpeg,mp2enc -F mpeg2video \
          -o giant

followed by:

mplex -f9 -o final.vob giant.m2v giant.mpa

please note that the extsub= has to be tweaked for each film, normally,
the last for positions.
