While radio stations have used both audible and inaudible cues--I
don't think the major tv networks have ever.

I know that NBC could "trigger" a local video insert id at the
beginning of a program segment (inserted by the local station from nbc
equipment or equipment adapted to "read" the signal) but I don't think
the system was ever used or designed to trigger local ad insertions.

Either the ads are programmed in advance (if the station is automated)
by pre-known times OR the operator watches for a specific promotional
announcement that runs a specfic length.  At one time -- CBS ids were
23 seconds--20 seconds of promo and 3 secs of cbs eye, ABC would use
ids as short as 5 seconds but now all the lengths seem to vary but
stick around about 20 seconds.

On the cable networks--those ads are triggered by an inaudible signal
-- years ago DTMF (I think that's what they were called) were used to
start, indicate length of break and end for cable stations.
