You need to rewatch BTE then. Kochanski is mentioned and they even talk about how and why she left (Lister believes she is dead. Kryten knows the truth. Chloe Annett played herself in BTE). They don't mention how Rimmer became a hologram again, but the assumption is that this took place in the fictional series IX that BTE talks about. They one they never actually made.
