If, by chance, you are using UniObjects, there is a bug in it, fixed at 10.2.22, that can cause this problem when a session times out or otherwise exits in a less than optimal manner.

I think that a corrupt file or index can also cause this problem

Alternately, you might check to see if there is anything in {uvhome}/ errlog.
