I have been seeing this bug on and off for at least several months, if not a year. It is very intermittent, but when it does happen it is painfully annoying especially because I usually must restart FF completely to resolve the issue.
I have seen this defect on multiple different versions of Ubuntu (9.04, 10.04, 10.10) with different versions of Firefox (but probably all 3.0+) as well as on Windows Vista, all with various add-ons installed. As other users said above, this appears to be a Firefox autofill problem.
