Nice clean up of hotline definition.

When first hitting "Add…" The *Label* and *Description* are empty, I think we 
could populate them as the attribute is chosen to reflect what they will be 
when OK is pressed? When using "Edit…" to reopen a created hotline definition 
the label is filled in.

[Naz] Added setting of label value on change of the attribute combo box. I left 
description as is as it is optional.
That works well, however when I supply a label (when editing an action), the 
label reverts to "State" when I go to edit the action later.
[Naz] Updated the dialog to only fill in the label if it is blank.


- I was able to hit OK and save it out without an action being defined.

[Naz] An error should occur when action is blank. I can’t seem to replicate the 
case above. Any tips?
1. From australia.shp sample file
2. In Document Property Page hit "Add…"
3. Choose Type "Action"
4. Choose Attribute "STATE"
5. press "OK"
6. Error message pops up, the OK button should be disabled instead

have been filled up.

Did not see that (when hitting "Edit…") it still shows up with the "question 
mark" icon used to prompt for information. In any case no rush, marking a tasks 
for stage 4.
[Naz] Will need help creating the images for these. My artistic skills are not 
good enough to make these.. :D
