I use Helplogic by Electric Butterfly (man I wish I had chosen THAT name before they did). It can build chm and apple files, plus they have a plugin for Realbasic (I know) if you happen to use that language. I think their website is www.ebutterfly.com if memory serves but it is a commercial product.
