by "hand-crafting" I was thinking of the selection of elements being displayed, level of detail and of course layout.

IMHO, a good UML diagram needs to be hand-crafted carefully in order to be helpful. If done well, it can provide good and quick understanding of a complex object model. Just my 2 Cents :-)
Are you referring to just the layout that needs to be hand-crafted or other differences between generated and hand-crafted UML diagrams? I never felt the need for a UML diagram myself. Having an IDE that provides easy navigation between subclass/superclass and interface/implementation relations plus Javadoc with descriptions and examples makes all the difference for me.
