I emailed the list about this about a month and a half ago, but got no response.

We had apparently ended up with many corrupted nodes in the workflow area, possibly when there was a JVM crash on the site one day (the exceptions seemed to start around the time of the crash). On the user end, it just appeared as though items were being sent to the Inbox and then never appeared, but behind the scenes we were generating several hundred thousand lines of ItemNotFoundException stack traces a day.

We ended up taking some drastic steps to fix it in our repository. I can't say for certain if this will help your site (and back everything up if you try it!) but we ended up: dropping all expressions_* and store_* tables (8 in total) from our MySQL authoring database, deleting all Lucene indexes for Expressions and Store workspaces from the authoring webapp, and restarting the site and letting it bootstrap and recreate the expressions and store tables again.

I don't know if this was the proper thing to do, nor do I know the ramifications of deleting those tables entirely, but we had to do something because the site was becoming unusable and it was a production site. Things have been ok since then, knock on wood, aside from a warning message anytime something goes through the Inbox now:

The quick "fix" for this problem is to export that page's content, move the page to a trash area and re-import the page. It works for a while but eventually this same thing happens again.

Can anything be done to make this problem go away?
