The re-opened bug report above got closed so I've opened a new one:

I've done a bit more digging and included the results in the bug history.

On Windows you should be able to fix this by renaming
registrymodifications.xcu (%APPDATA%\LibreOffice\3\user). However
other application settings will be reset by doing this (which is why I
say "rename" and not "delete"!).
