mod_proxy should add the "X-Forwarded-Host" header (i.e. HTTP_X_FORWARDED_HOST in cgi/php) with the value of defaulthost, when forwarding the request to proxyhost. This is the default behaviour, unless "ProxyAddHeaders off".
