Please check this first:

Insert->Cross Reference->References
do you see a "Illustration" field under the Type box? If so, every time you 
attach a figure/drawing/picture to your document, you should use the 
"Caption" command to add a description to it. When selecting the Cateory for 
the caption, make sure you select "Illustration". That way your figure will 
be linked to the Illustration variable to keep count.

When you make a List of Figures, select "Illustration" as the table type, 
and NOT the default "Table of Contents". OOo will then create the table 
correctly.
