Mail merge is the wrong concept for what you want to do. Try Labels instead.

Mail merge takes each database record and creates a separate document from it based on the template. (Think form letters)

Labels takes each database record and places that info in a separate frame on a page, creating as many pages as necessary. It's assumed that the frames are identical. (Think a sheet of mailing labels or convention name tags)

Finally, you can create a database report that is basically an organized data puke. (Think bank statement or inventory report)

The reason you got the results you did was that the next record statements in the other imbedded frames told OOo to skip those succeeding records.

So what you can do is go to File -> New -> Labels and let the Wizard walk you through it. You can either use one of a few hundred pre-defined label formats (Avery, mostly) or you can define your own either from scratch or by modifying an existing one that's close to what you want.
