I did some experiments, and you can reproduce some of this, but you end up doing it manually. This is very tedious. I created my own numbering fields, and then I incremented the numbers in the side columns.

I will send an example off list, since I can not send an example on list :-) Hmm, no, perhaps I will simply post it on my web site. I created this documenting using OOo 3.0 dev build, but it will probably load fine into 2.4.1.

One could work with columns, but any time the page or font size was
changed, it would disturb the formatting.

I used to be able to do this in Ventura (remember that?) by manipulating
text margins and using a paragraph style without the return inserted at
the end of the paragraph. If I could do that with a paragraph style in
OOo, that would also work, but I've not seen how to remove it.

Also, I assume there's no way to have text in a single column and
footnotes in a two-column format?
