Could be almost anything causing your problem. A couple example X-Spam-Statuses might be a start.. Beyond that, well, if you're running anything older than 2.64 you're probably so outdated that modern obfuscation techniques are effective against your version of SA. Realistically, SA needs to be updated about once a year, unless you're putting a lot of hand-tuning and custom-rule writing into it.
