It's up to your MTA to decide what to do with the mail once SA or CLAM
have flagged it. In itself CLAM is just an AV scanner the same way SA
is 'just' a spam scanner. Once a mail has been flagged as spam or
virus your MTA should take over from there and decide whether to drop
the mail, deliver it with warning or some other action.

We use CLAM inline here but it only returns whether a message is clean
or not, depending on that the MTA delivers it or dumps it into the
virus holding directory on the mail server.
