I assume you mean that the SCORE indicates that it is not spam, right? Do you get the same results when you run the message through spamc? If it is marked as spam when it goes through your mail system, but is not marked as spam when you do a manual test using the same settings, then obviously your mail system is doing something else. What user were you logged in as when you ran the manual test? Are you sure this is the same user exim is using? Show us the full message that was marked as spam and delivered including all headers.
