Yes,

Here is the URL I'm trying: //eptest01:8080/manager/html which uses the
direct port.

And I copied the same tomcat-users.xml from Tomcat 4.1.12 which works
great.

What happens if you try Tomcat's direct http port (usually 8080), rather
than going through mod_jk?

Have you updated conf/tomcat-users.xml to include a userid with the
"manager" role?
