I can't help on actual deployment numbers, sorry.

The one I personally operate is small enough to not need
VPS-equivalents;  I'm tempted to suggest that the combination
of DNS support (MX vs. A records) and the richness of
hop-by-hop email processing possibilities (developed ad-hoc
before and during the transition to the Internet) combined
with the less-harsh latency requirements on mail vs. browsing
make VPS' for email not needed.

As you say, the feature is there, in Exim.
It's agnostic about port-number and the purpose of the service.
