I spent most of yesterday going through all the documentation bits in the
utrace patch.  The Documentation/utrace.txt file is gone now, and
everything is in DocBook instead.

I went through all the text and rewrote bits to match the new interface.
I reviewed and updated all the kerneldoc comments.  The 'make htmldocs'
pages look decent (I didn't look at other output formats).

Please review all this text and give me any feedback ASAP.

This cosmetic update was the main thing I wanted to finish before
submitting the code upstream, now that I've sent tracehook.

Of course, I'll be looking at the recent bugs too.  But I expect more
upstream feedback about the trivia and cosmetic issues than anything
actually subtle that requires really thinking about using the interface as
folks here do.  The aesthetics will be the likely barrier to acceptance,
not whether the new experimental code is still buggy.
