Courier does this - your mail is all in one Maildir hierarchy.
courier-imap isn't a mail client, though, it's an IMAP server. Most mail
clients (MUAs) will show folders separate from messages. It
sounds like maybe you want a folder to show up as a special sort of
message in your client? Or are you only concerned with the directory
structure on the server?
