well if the machine booted a couple months ago, then the drive probably works fine, but you never know. its probably a good idea to clean it anyway though.
if you have extra floppy disks around, you could try downloading a boot disk (Google for an old version, System 6 and newer won't work afaik) and writing it out to a disk using the Quadra, and seeing if the 128k boots from that. if it does, then maybe the System 1 disk you got with the 128k somehow got messed up (its possible). if you only have HD (1.44MB) disks then you can use something sticky to cover the hole opposite the write protect hole, that will make the Quadra think it is a DD (400k/800k) disk. (that trick should work fine temporarily, but don't rely on it for prolonged usage.)
