I don't have it handy but it is in the apple site it is easy to find there

The problem is that the individual disk images don't fit on a floppy
formatted on a new Mac

I got around it by copying the images on a PC formatted floppy and reading
them with PC exchange on my SE30. If you have a CD rom you can use that also
