Somewhere, somehow, get access to a PC with a SCSI controller that has a BIOS on it. (Ie, can boot from SCSI hard drives.) The controller should be able to low level format the drive. Usually during bootup there will be a message on screen to press a couple of keys to access the SCSI BIOS. Ctrl + A for Adaptech, Ctrl + I for Initio controllers. If that won't touch the drive, toss it. :) I have had drives so screwed up that none of my pretty large collection of Mac formatting tools would touch them until I "nuked" them with my PC's SCSI controller.
