When NOT using any external SCSI devices, you don't need a terminator on the external connector (except it helps on the early IIfx). Since you're using an external CD-ROM it must have a terminator on it. You should not have more than one internal and one external device providing termination power to the bus. SCSI devices usually have three options for termination power, to themselves only, to the bus only or to both. Some devices are configured so that one jumper both provides termination power to the device and enables the onboard terminator. Desktop Mac's built in SCSI is supposed to provide termination power to the bus so that when using only one internal drive and one external device, neither should need to provide termination power. But it looks like some Macs may not provide enough or it's failed completely. So to be sure, always have one internal and one external drive provide termination power to the bus and themselves and all others set to provide their own (unless that setting also enables their onboard terminators). That makes it easier to change things around. ;) The position in the chain of the device providing termination power to the bus doesn't matter, as long as you have one on each side of the controller.
