First when I am answering the phone on the sly, often there is a lapse in VO
reading me buttons for accepting, declining, etc. I do not leave the phone
app open while I am doing other things so I am rarely in the phone app when
calls come in. 

When on a call that I've asked Siri to dial, and it rings, that is when
there is a notable lag in when vo reads the numbers on the keypad. I'm not
always in phone when I call someone back, and often  I simply ask Siri to
dial for me. Is it then that I need to quickly get to the "in call" item so
I can hit the menu item that I want? Using the app switcher as one person
suggested is a bit slow and hangs things up. 

Did you really mean to say that there is a way to tap on the status bar at
the top of a home screen to get to in calling? If so, I did not know that.

 

And I am really confused by your description below of what to do to pause
Siri. 

You say to hold the button down the whole time you are speaking to pause
Siri. What is the point of speaking if you are wanting to pause? See below.

Note that the second item can be a source of frustration.. If you're holding
the button down too long, when you release it, Siri could think you just
issued a very short command. This is why Siri sometimes seems to stop
listening almost immediately. It's a matter of practice to get the timing
down right, but again, if long pauses are your problem, yo might find this
feature quite helpful.
