Aside from checking with, and complaining to, your cell provider, could it be 
VoiceOver? If you use VO on calls a lot, it could be causing this, since your 
microphone mutes itself whenever VoiceOver speaks. That prevents VO speech from 
being sent through the call, but it also prevents you from being heard if you 
speak at the same time. Just a thought.
