A good way to know if a book has accessibility issues is to look at the number 
of pages as compared to the size of the book. If it is a story that seems like 
a novel, look up chrono trigger in iBooks for an example of this, the first 
result is the one I'm talking about The book had only 43 pages. No novel is 
ever that short, so I was very sure it had inaccessible immages. I do think 
that books on math and such should be in math-ML so that voiceover can read it 
properly, but that's just me and my frustration in high school for finding good 
math-teaching books. I've also tried some free comic books, which are also 
inaccessible, sadly. "image, image, image" is all they contain.
