Hi Ari,

The Automatically Add to iTunes folder is simply an easy way to put files like 
music or ringtone files into the folder where they should be and at the same 
time to add them to the appropriate library. Keep in mind that just copying a 
rington into the ringtone folder or a song into the music folder does not mean 
these files are also added to the library which is to say if you just copy a 
song into the music folder and then open iTunes, that song is not yet in your 
list of music. You have to sync or update that list so to speak by going to 
File > Add file to library or File > add folder to library and select either a 
single file or an entire folder. If, however, you copy the song or ringtone to 
the Automatically Add to iTunes folder, iTunes will do this automatically. It 
will know by the extension of the file where it should go.

The only time where this would not be of use is if you completely maintain all 
your media on an external drive or if you have it on the hard drive, but for 
some reason choose not to use the subfolders in the iTunes Media folder. I can 
see where somebody who has maybe a not very large solid state drive in their 
laptop may want or need to use an external drive if they have a ton of music or 
even more so for movies, but I don't see why one would not put ringtones in the 
default folder since they don't take up much space.

I don't think this automatically add to iTunes folder will work well with a 
library that's completely managed by the user instead of by iTunes.  Is this 
assumption incorrect?
