If they're a Jaws user, then there shouldn't be a problem. I do use iTunes for backing up and major updates and other stuff in general, it's just not a pleasant or efficient experience. No clue about NVDA, but I'd imagine the experience can't be too different. There are also Jaws scripts to "enhance" the screen reader experience, but they're not essential to interact with iTunes. There's just a lot of tabbing back and forth involved.

The specific context of my friends contacting the Apple people had to do with her iPhone, which died, and which she was apparently trying to restore from iTunes. She does not have window eyes. And her local cited help is not particularly screen reader competent either. So she ended up on the phone with the Apple folks. And that's when the senior person told her that window eyes was the only game in town.
