Sorry about the delay in responding, but I wasn't totally sure of
the answer myself, and was thus thinking someone else might have
answered. But, I finally got out my iPhone 6S and decided to
give it a try. And, bottom line, there was an option to send the
code to my cell phone number; and, I'm guessing because the SIM
card was in and all, the device just got recognized. Now, I
could have set up another device ahead of time to be called with
a code, such as my home phone, but I elected not to do so as of
now.
Anyway, if you want to hear this for yourself, such as it is, I
used my Braille Sense notetaker to record what happened with the
two-factor authentication in my case. I also recorded, as I just
wanted to know for myself, just what happened when I tried to
activate the phone without a SIM card. Below is a Dropbox link
that I'll leave up for a couple of days, in case you are
interested. But, please remember, that I only do this kind of
thing just for fun, and it is not even close to a professional or
experienced recording:

Hi Vicky and everyone else
I had been looking into this over the past week, as I'm
preparing a short presentation on security for our iPhone Users'
Group. I am ashamed to say 2 Factor Authentication is about the
only thing I don't do and I know I should.
What I would like to know is what happens when you are trying to
set up a new iPhone, and are swapping over the sim from the old
one? How can you get the authentication code then? Presumably the
sim will be in the new device, so you can't receive the SMS on
it because it hasn't been authenticated.
My brain hurts.
