I’ve also seen the same issue several times but in my case, it’s easily fixed by using Siri to turn VO off then back on again. So far that’s worked for me every time.

Yes, it can be a bit tricky sometimes, but at least it does work if you persevere. I have also found recently that sounds on and off do not seem to be terribly consistent. Sometimes, when I put them on, they don't come on and vice versa! For all those who have had to reinstall apps because they have lost their speech, I would suggest you persevere with trying to get the speech back. Plugging the phone in first may well help. We all ought to start reporting this, I think!
