What you describe about the app starting to read text over and over sounds like 
you are moving the phone/camera.
I am getting very good results as well, this morning I received my neww 
government guide dog Id card in the mail and I pointed the phone at the letter 
that came with it using the short text function and it read it out flawlessly.
I just tried it again pointing my iPhone 6S Plus towards my 24 inch LCD screen 
from about a foot away with Outlook and this email open and it read a bunch of 
the menus and things on the screen and then most of what I just wrote here.
Also just tried it by holding it over my Apple Watch where it read me the date, 
time and the current temperature which are some of the complications I have on 
this watch face. Only mistake was that it said "230 Fair" instead of "23 
degrees Fair", so it recognized the degree symbol as a zero.

I'll give an example. Let's say I'm trying to view text from a scanned PDF on 
my computer. I hold the phone centered on the screen, about a foot away. All it 
will read me is the stuff at the top of the screen, with the menu bars and such.
