Hello Wendy,
As to the part where no one hears her, I would confirm that the
microphone is not muted. To do this, answer the call and take the
phone away from the ear to envoke the speaker phone if she is a
VoiceOver user and flick to the hide keypad button along the bottom
and double tap on it. Next, flick to find the mute button and make
sure that it is not selected. If it's selected double tap to unmute.
As to the part where she is not able to hear anyone I would try
turning up the volume using the volume up button  at the left side of
the phone. If she is still not able to hear anyone I would go to Apple
 to have it checked out.
