Have you tried to raise the volume when the phone is on speaker? Often when you get Voiceover and such on the headphone and not on the speaker it's because speaker volume and headphone volume are independent from each other. Your speaker volume may simply have dropped to 0%. I would try to unlock your phone using the headset, then when you are on the home screen on the top left icon, unplug the headset and do a 2-finger swipe up or down to start Voiceover reading, then repeatedly press the volume up button.
