Go to Settings > General > Accessibility > Voiceover > Braille. Check to see what Braille Input is set to; perhaps you accidentally switched it to eight-dot Computer Braille. If you did, switch it back to either uncontracted or contracted six-dot, whichever you prefer. Just a thought: let us know what happens. Oh, and if anybody knows what command you use to switch Braille input on the fly, please post it here so that I can make a note of it.
