General speaking, when you plug something in to the earphone hole, it'll
disable a bleutooth headset, earpiece, etc.

Of course when the earphone hole has something in it. You're not going to
have voiceover at all. I don't use the card reader. I just inter the
credit card numbers by hand. The only down side to this is, you'd better
trust that the person who is reading their card number to you is actually
being honest about it. However, I've yet to find another work around.

I'm setting up a Go Payment account to accept credit card payments and it
seems to bipass the iPhone speaker and VoiceOver only comes through the
part you hold to your ear.
