These are peculiar to having zoom and Vo on at the same time.

On my iPhone 4 S, running IOS 6, the three finger double tap toggles
Zoom on and off. It is a three finger triple tap that toggles speech
on and off.

BTW ... A three finger quadruple tap toggles the screen curtain.
