I don’t have any problem with VoiceOver, and it only seems to occur when I am connected to Bluetooth for a speaker or for AirPods. I can cast some thing to my TV without there being an issue, but it’s weird whenever it’s connected to Bluetooth for audio. Only, it seems to be an issue. It was happening before the release of iOS 16.5, so I don’t think it’s a version issue at least not with the latest version anyway. I’ve never had any issue with voice over at all, so I can’t say there’s anything there either.

Have you tried turning the bluetooth off and then back on again?  Or try turning off the phone and then turning it back on again.  Those things can sometimes fix weird issues that pop up.

I would be interested to find out if anyone else has run into this issue and if so, what may have been done to correct it short of if it all possible, resetting the device to factory defaults and erasing all data and contacts.
