For most people, the screens of the watch are asleep.  This is why notifications are not automatically read out, and this is why you have to touch the screen to get the time or any information from the complications on your watch. So when a notification comes in, your watch will make a ding sound, then if you touch the face of the watch, it wakes up the screen and makes the notification so you can access it and read it. there are 2 settings that could have an effect o'n this.  In the watch app, in display and brightness, the 2 settings are; wake on wrist raise, and the always on setting.  If either of these settings are on, then this might help notifications to be automatically read out sometimes.  I have not tried this myself, so I can not say for sure.
