It is not possible to record a phone call when the iPhone is up to your ear. Apple does not allow any app to access the phone channel.
That said, it has been reported that you can record phone calls, using some apps, when the phone is in speakerphone mode. Apparently, the mic is used to record the sound from the speaker. I have not tried it, so cannot attest to whether it works or which recording apps to use. The reason Apple blocks direct access to the phone communication channel is that it is illegal to record phone calls in many jurisdictions.
