As Joe suggest, if you buy Groundwire from the developer Acrobits and have a
VoIP provider which you set up in Groundwire you can call somebody and once
the call started you can simply press the Record button. By default there
will be a beep every 15 seconds since in many juristictions it is illegal to
record phone calls or at least to record them without letting the other
party know.

This is also the reason why Apple does not allow apps in the app store which
can do this easily with a regular phone call. There are apps out there, but
most of them involve a subscription to some service where if you want to
record an incoming call, you would have to place the caller on hold, then
start the app which would call a server and then the calls are merged and
your call is recorded on a server somewhere.
