Just as an endorsement, Groundwire is definitely a fantastic app and
completely accessible. It's a good buy at $4.99 and anybody who is using
VoIP calling would probably not regret getting it.
As Esther said, these apps all require subscriptions to VoIP providers and
there are too many out there to list them, but Groundwire also supports
incoming calls if you have an incoming VoIP number and it even works if the
app is not in the app switcher. In this case you get a push notification
which allows you to answer the call.
The app is very much like the stock Phone app with a Favorites tab called
Quickdial, a recents tab called History, a keypad and access to your
contacts list. You can also record both sides of an ongoing phone call, the
Record button is just there along with buttons for Speakerphone, keypad etc.
One thing Groundwire does not offer yet is touch dialing, e.g. you have to
double or split tab the numbers and I find it often does not work well for
dialing access numbers or entering account numbers.
Skype currently has the edge here since to my knowledge it is the only app
where touch typing works on the keypad although it also only works on the
main keypad, if you are already on a call and bring up the keypad to enter
numbers, then once again you have to double or split tab.

The first two apps are known to be accessible, and have entries at AppleVis.
The iPad version is a relatively recent release from a few months ago, but
has been through three updates, and I don't know details of its
accessibility. However, this capability of making calls through the iPad may
be of interest to some users.  Note that you must have a service providers
hat supports these capabilities for this to work.
