Hi Charlene,
I have been able to use List recorder to record phone calls. It is not
able to directly record but will work if you have your phone in speaker mode.
It can recored the sound coming out of the speaker and your voice. It should
be good enough to take notes from.

If you want to do that then you will need List Recorder full as the non full
version has only 10 second recordings until you do a in app purchase to make it
a Full version. Purchasing List Recorder Full saves you a dollar.

This is a recording and note taking app with strong organizational capability

This is how I use it to record a phone call.

I first start list recorder and place the focus in the list I want the
recording to be placed.

I then press the home button to leave list recorder. Once the call is started
I move the phone away from my ear and then press the home button twice to bring
up the app switcher.
I select list recorder.
I then start a recording.
I continue the call in speaker phone mode so that List Recorder can record the
out put from the speaker and my responses.
You can leave the app and go back to the phone or into another app and it will
continue to record unless the new app takes over the microphone.
The List Recorder app is completely voiceOver accessible
as it was designed from the ground up with the blind user in mind.

I hope this is helpful.
