First, start the app and get to the point where you are ready to double tap the 
take picture button, and plan to ignore what it says about positioning the 
camera.
Scanning free-hand is tricky at best.
You want to use the back camera.
I'd recommend placing the phone flat on the page with the back camera roughly 
in the middle of the page with the home button pointing to the bottom of the 
page and the power button pointing to the top.
Grab the phone with your thumbs and little finger holding each side near the 
top and bottom.
I have my left thumb over the mute switch if that helps explain the location.
Then, lift the phone about 10 to 12 inches above the page, assuming an 8.5 by 
11 page.
Try and keep the phone as steady and level as you can.
Then, using your right middle or index finger, double tap the take picture 
button.
You should hear the rising tones before the picture snaps.
After you have gotten used to hovering the phone over the page this all gets 
easier.
