You should be able to if Comcast offers cable cards. I'm not sure if it is a requirement that cable companies have to offer the cards, but it seems like many have the option, even though it may be buried. I know with Time Warner, the option seems to be right there when you hit order or upgrade equipment in their account once you log in. No you do not need the stream to start shows playing on your TV by the app. It's fantastic as I use the app for everything. You will need some sighted assistance the first time you set the box up, but after that it is very rare that I need sighted assistance ever with my Tivo. Of course, not being able to see you can't use all the features, but it is the best option on the market that I have found as the app is mostly accessible. I have posted here before with the app, I can view my entire tV guide by time or channel, search for any shows and set a season pass or record by episode, you can see upcoming episodes, edit your season pass list, start programs playing on your TV by the iPhone, it's pretty fantastic and I use it every day. With the Stream you can be on any Wifi network and connect to your Tivo to watch anything on it.
