Go to:
https://appleid.apple.com/
Then, click the "Create Apple ID button." Everything is pretty much
straight forward (name, E-Mail Address, what you want your Apple ID to
be, ETC.)
There is a captcha you have to fill out, but there is an audio captcha
available. You find this by clicking on the link that says "Visual
Impaired."

Not only that it has acaptcha, but also the check box for accepting
the terms and conditions is invisible to screen readers. Any idea on
